What is The Complete Process of UDID Card?

To understand the complete process of the UDID card, read the following points. This will help you to understand the whole process with hierarchy

  • Step 1: Complete the registration process by following all the steps mentioned above.
  • Step 2: When you complete the application submission process, your application is sent to the Chief Medical Officer (CMO) or District Medical Officer (DMO). They verify your information.
  • Step 3: The CMO, DMO office, or Medical Authority assigns the concerned specialist doctor to obtain an opinion.
  • Step 4: The specialist doctor assesses the PWD’s disability and submits the assessment report to the medical board.
  • Step 5: The medical board will determine the type and percentage of disability, as well as the validity of the disability certificate.
  • Step 6: The CMO or DMO will inform the board of their decision. The CMO Office prepares the Disability Certificate and generates the UDID.
  • Step 7: The UDID data sheet is used to print the UDID Card, which is then dispatched to the PwD.
  • Step 8: The card will be sent to the applicant.

How can I renew my expired UDID card?

What are the different types of UDID cards?

Yellow Card

What is the validity of a UDID card?

  • Please note that if there are corrections to the name, date of birth, photo, address, or similar details, no appeal is required. The PwD can use the mechanism after logging into the Swavlamban portal, and reassessment is not required.
  • This SOP is only for grievances related to original disability certificates and UDID cards. A separate SoP is being issued for appeals against the medical board’s decision when PwD applies for higher education and government jobs, etc., under the PwD reservation quota.
  • All offline appeal practices in a State/UT shall cease once that State/UT transitions to the online appeal process.
